A new study by the Institute of Alternative Futures (IAF) found that community health centers (CHCs) are the most adept at impacting the social determinants (i.e. lack of transportation, access to fresh foods, etc.) of a patient's health. This is because of CHCs' full integration into the communities they serve. Hospitals, physicians, and other care providers are encouraged to build strong partnerships with CHCs in order to leverage this strength.
