SCHCCN Program Overview

What is a HCCN?

Health Center Controlled Networks (HCCNs) exist to strengthen the operational, clinical, and technological capabilities of community health centers. HCCNs advance health equity by enabling data-driven decision-making, supporting sustainable health IT adoption, and promoting collaborative innovation across the primary care safety net. HCCNs serve as shared-service organizations that help member health centers solve challenges that are costly, complex, or inefficient to address independently. Through coordinated infrastructure, expertise, and resources, networks promote efficiency, compliance, quality improvement, and financial sustainability.

Who is the SCHCCN?

The South Carolina Health Center Controlled Network (SCHCCN) is a HRSA-funded entity that was formed in 2016 under the South Carolina Primary Health Care Association. The SCHCCN membership consists of seventeen (17) South Carolina Community Health Centers, two (2) Look-a-Likes, and a South Carolina Agriculture Worker Health Program.

SCHCCN’s mission is to strengthen Member Health Centers’ use of health IT and data analytics, supporting HRSA’s 2025–2028 HCCN focus areas:
